Clinical Commissioning Group
The Practice is within the area covered by NHS County Durham Clinical Commissioning Group. The CCG has been formed following the merger of NHS Durham Dales, Easington and Sedgefield CCG and the NHS North Durham CCG.
The newly formed CCG, serving a population of around 500,000 people, will retain a focus on local people and local services. This will be led by the Governing Body, which will continue to have representation of GP practices, lay membership for public and patient involvement, local clinical leaders and senior managers.

Sedgefield North Primary Care Network (PCN)
PCNs are groups of GP practices working closely together with other primary and community care staff and health and care organisation to provide integrated services to their local populations. These networks have been established to help deliver the NHS Long Term Plan.
PCNs will focus on the health needs of their particular patient population using information provided by Public Health, CCGs and from GP clinical systems. Over the next five years PCNs will employ additional staffto provide a wider range of services for their patients either at their GP practice or nearby. PCNs will work more closely with community services to support patients who need more than just health care.
There are 13 PCNs across County Durham. Each network has appointed a clinical director and during 2019/2020, will have access to funding for a clinical pharmacist and a social prescribing link worker, followed by funding for other roles from 2021 onwards.
Our PCN is Sedgefield 2 and the Clinical Director is Dr Matt Shield. Our PCN consists of some 56,000 patients from following Practices: St Andrew’s Medical Practice, Bishop Close Medical Practice, Ferryhill & Chilton Medical Practice, Skern Medical Group, West Cornforth Medical Practice.
